Change your registered name

Submit the 'Change of name request' form below attaching CERTIFIED COPIES of BOTH:
  1. Current photo ID, e.g. passport page or front and back of Australian driver's licence.
  2. Change of name documentation, e.g. certified front AND back of marriage certificate.

Update other details

Change your details via MyAccount: www.vetboard.vic.gov.au/myaccount

You must update addresses within 28 days of any change.
